Available in units of $10,000 to a maximum of $250,000, subject to approval by the insurance company for all amounts in excess of $20,000. If you are covered under this plan as both an employee and spouse, you are limited to the $250,000 maximum. The cost of coverage is based on age, gender and smoking status.
You will be required to provide evidence of your insurability (EOI) for amounts in excess of $20,000. At the end of the enrollment after you have hit the "Confirm" button you will be provided an opportunity to print out the EOI form if required. The form is required by the insurance company to determine whether you satisfy their health requirements. Once your approval has been received, your plan will be brought up to date and the coverage will be effective from that time forward.
