In this benefit you can choose Optional Life insurance and complete your beneficiary choices. The cost of the coverage is rate calculated and based on your age, sex and smoking status. You are required to fill in a beneficiary(s) for this benefit.
Coverage may require you to provide evidence of your insurability (EOI) as indicated by the heading [Evidence Required].A tab will appear if EOI is required called [Pending]. At the end of the enrollment after you have hit the « Confirm » button you will be provided the required EOI forms for printing. The form is required by the insurance company to determine whether you satisfy their health requirements.Once your approval has been established your plan will automatically be brought up to date and the coverage will be effective from that time forward. You will receive notice in writing from the insurance company.
