Select your life insurance

In this section you will choose your life insurance and complete your beneficiary designation. The first $10,000 of your Basic Life Insurance is a Death benefit payable directly by Resolute Forest Products. You must complete the beneficiary designation for this benefit. The cost of the coverage, up to 1 times your salary, is based on an average group rate. For units above 1 times your salary the rates are based on your age, gender and smoking status as of December 31st of the year preceding the year of coverage. Depending on the amount of coverage chosen, you may be required to provide evidence of your insurability (EOI) as indicated by the heading [Evidence Required]. A tab will appear if the EOI is required called [EOI Pending]. At the end of the enrollment, after you have hit the "Confirm" button, you will be provided the required EOI forms for printing. The form is required by the insurance company to determine whether you satisfy their health requirements. The additional coverage you requested will take effect after the insurance company has approved the EOI and will be subject to payroll deductions. You will receive notice in writing from the insurer.
