If your spouse has coverage, indicate the co-ordinating benefits with your spouse's planby checking the dependents covered under another plan check box. Benefits paid will not exceed 100% of the eligible claim. The insurer has the right to receive and release information on benefits and, if necessary, collect any overpayments.
