Optional Spousal Life Insurance.

The cost of the coverage is based on Spouse's age, gender and smoking status.

In this section you can choose from a number of life insurance options for your Spouse. The cost of the coverage is based on Spouse's age, gender and smoking status. Depending on the amount of coverage chosen, you may be required to provide evidence of their insurability (EOI) as indicated by the heading [Evidence Required]. A tab will appear if the EOI is required called [EOI Pending]. At the end of the enrollment, after you have hit the "Confirm" button, you will be provided the required EOI forms for printing. The form is required by the insurance company to determine whether your Spouse satisfies their health requirements. The additional coverage you requested will take effect after the insurance company has approved the EOI and will be subject to payroll deductions.
