This page displays the options available and allows you to get the forms and other information which might be helpful during the enrollment period and all year round.
Click on the desired option. You can cancel what you are doing during the session and continue later on. When you are satisfied of your choices or changes that you have made,
you must confirm to save the new information.
Life Event refers to major changes in your life, like getting married, having a child, gain or loss of coverage for your spouse, etc. You can enter a Life Event Change at
any time during the year. The effective date must be within the last 30 days, you cannot enter a date older than that.
