This page provides you with the opportunity to review your
Dental coverage options.
Your Dental option is linked to the choice you select in
Health – if you wish to have a different option here you must change it under
the Health Benefit.
If you click on the coverage descriptions underlined in blue
you will be provided with a brief description about that option. Detailed
information is provided in your "Choices Plan Details and Enrollment
Guide". If your spouse has coverage
indicate the co-ordinate benefits with your spouse's plan by checking the
dependents covered under another plan check box under Health and Dental. Benefits paid will not
exceed 100% of the eligible claim. The insurer has the right to receive and
release information on benefits and, if necessary, collect any overpayments
made by it.
