Optional Critical Illness (Employee and Spousal)

Your Choices Benefits Plan offers you the opportunity to purchase Optional Critical Illness Insurance. The plan is designed to provide you with a pre-determined Lump Sum Payment from $10,000 to $100,000 should you be diagnosed with one of the specific conditions. The coverage is available to the employee and your eligible spouse.
