The Choices Benefit Plan offers you the life insurance
coverage in the event of your spouse's death from $10,000 to $250,000 of coverage in units of
$10,000.
The selection of some options require you to provide evidence of your insurability (EOI).
At the end of the enrollment after you have hit the « Confirm » button you will be provided an opportunity
to print out the required Evidence of Insurability (EOI) forms.Complete the form and send directly to the insurer as instructed.
While waiting the approval the system will automatically enroll you in an option that does not require evidence of insurability. Once your approval has been received, you will be enrolled in the plan effective from the date of insurer approval.
