Your choice of protection for health and dental care correspond to the options listed in the table below:

For more information about the different options, please refer to the Benefits booklet available on this site under the «Plan Details» section.
If you are covered under your spouse's health care coverage, you can opt out of this benefit. Click on the «Waiver Form» and provide the information of your spouse's plan. Please note that by opting out of the Health Care benefit, you are also opting out of the Dental benefit.
Termination of benefit: Your retirement or your 70th birthday, whichever comes first
If you choose Module B or Module C, you must keep it for a period of 2 contract years before you can reduce your coverage.
