Coverage options and make your
selection.
For more information on the different levels of coverage, please refer to
the Benefits booklet available on this site under the «Plan Details»
section.
If you are covered under your spouse’s medical plan, you may opt out of the
Health Care coverage. Click on the «Waiver Form» and provide the
information of your spouse's plan.
Termination of benefit: Retirement or termination of employment, whichever comes first.
